How to schedule an appointment
Please call (978) 462-7555 and one of our secretaries will book an appointment for you. If you are calling after office hours, please leave a detailed message, including your name and phone number, and someone will return your call on the next business day.
What to bring to your appointment
For all patients:
Please bring your health insurance card, a written list of your prescription medications and any necessary referral information. Additionally, Massachusetts law requires a form of ID for the patient or, if a minor, the guardian will need to have proof of identity.
Please bring the Patient History Form, which you can complete and print directly from our website. To access the form, click the blue link found in the previous sentence or under the “Online Forms” tab at the top of any page. Please note that you can type directly onto the page and then print out the form.
For workers compensation injuries:
Please bring the Workers Compensation Approval Form, which may be accessed by clicking the blue link found at the beginning of this sentence or under the “Online Forms” tab at the top of any page. You can type directly onto the page and then print out the form.
For patients being seen for knee/ hip pain:
Please bring a pair of shorts if possible.
For patients being seen for back pain:
Please bring a pair of sweatpants or shorts/pants without a zipper.
Any applicable copayments will be collected at the time of your visit. In addition to cash, checks and debit cards, we accept Master Card, Visa, AMEX, and Discover.
Appointment Cancellation Policy
If you need to cancel your office visit, please be sure to call within 24 hours of your appointment so that we may offer the time slot to someone on our waiting list. You can reach the office at (978) 462-7555.
